ZivaDoc is not a healthcare provider, hospital, medical practice, insurance company, or employer of healthcare professionals unless expressly stated.
Healthcare services are provided solely by the independent healthcare provider selected by the patient.

While ZivaDoc makes reasonable efforts to maintain accurate information, we do not guarantee that any listing is complete, current, or error-free.
Patients are encouraged to verify provider credentials, insurance participation, office policies, and appointment availability directly with the provider before receiving healthcare services.
Listing on ZivaDoc does not constitute an endorsement, recommendation, certification, or guarantee of any healthcare provider.

8. Appointment Booking

ZivaDoc provides an online platform that enables patients to search for healthcare providers and request or schedule appointments.
Appointment availability displayed on the platform is based on information provided by participating healthcare providers and may change without notice.
A booking request submitted through ZivaDoc does not guarantee an appointment until it has been confirmed by the healthcare provider or through ZivaDoc's appointment scheduling system.
Healthcare providers reserve the right to accept, decline, reschedule, or cancel appointment requests due to provider availability, emergencies, scheduling conflicts, or other operational reasons.
Patients are responsible for reviewing appointment details and ensuring they arrive at the scheduled location on time.

9. Appointment Cancellation, Rescheduling, and No-Shows

Patients may cancel or reschedule appointments through the ZivaDoc platform or by contacting the healthcare provider directly, subject to the provider's cancellation policy.
Some providers may require advance notice before cancelling or rescheduling an appointment. Failure to provide sufficient notice may result in cancellation fees or restrictions on future bookings.
Healthcare providers may cancel or reschedule appointments due to emergencies, scheduling changes, or unforeseen circumstances. When possible, patients will be notified promptly.
Repeated missed appointments, late cancellations, abusive behavior, or misuse of the booking system may result in temporary suspension or permanent termination of a user's account.
ZivaDoc is not responsible for any costs, damages, inconvenience, travel expenses, lost wages, or other losses resulting from appointment cancellations or scheduling changes.

10. Payments and Billing

Certain healthcare providers may offer online payment services through ZivaDoc.
By making a payment through the platform, you authorize ZivaDoc and its payment processing partners to process your payment using your selected payment method.
Patients agree to provide valid payment information and authorize charges for healthcare services, consultation fees, deposits, cancellation fees, or other applicable charges.
Healthcare providers remain solely responsible for determining consultation fees, procedure costs, and other medical charges.
ZivaDoc does not determine, negotiate, or control provider pricing.

11. Refund Policy

Refund eligibility is determined by the healthcare provider's cancellation and refund policies unless otherwise stated by ZivaDoc.
Where ZivaDoc collects payments on behalf of providers, approved refunds will generally be processed using the original payment method.
Processing times for refunds depend upon the payment processor and financial institution.
Platform service fees, payment processing fees, or convenience fees may be non-refundable where permitted by applicable law.

ZivaDoc may display insurance plans accepted by participating healthcare providers based on information supplied by providers, insurance companies, or third-party data sources.

A healthcare provider-patient relationship is established only when a licensed healthcare provider agrees to evaluate or treat you in accordance with applicable laws and professional standards. ZivaDoc acts solely as a technology platform that facilitates communication and appointment scheduling between patients and independent healthcare providers.

17. Healthcare Provider Independence

Healthcare providers listed on ZivaDoc are independent professionals or organizations. Unless expressly stated otherwise, they are not employees, agents, representatives, partners, or affiliates of ZivaDoc. Healthcare providers exercise independent professional judgment regarding diagnosis, treatment, prescriptions, referrals, procedures, pricing, scheduling, and all clinical decisions. ZivaDoc does not supervise, direct, or control the medical services provided by healthcare professionals. Accordingly, ZivaDoc assumes no responsibility for the quality, safety, effectiveness, or outcome of any healthcare services provided by independent healthcare providers.
